Attire - Men please dress in business casual and ties are encouraged. Girls please wear a dress and get as dressed up as you would like!

Daughters-Ages 3-18

$10.00 per couple, ($15.00 max for extra daughters.) 

*Please invite your grandfather, uncle or other important male figure if dads aren't available. 

Either the parent or the young lady must be a member of Community Bible Church. If invited by CBC member, please indicate by checking the box on the form.
